Taylor Swift’s “Patient Zero” Music Video Makes So Many Subtle Nods to Her Past Work

It was the Taylor Swift show at the 2026 MTV Video Music Awards last night, where the pop superstar not only won Video of the Year for “The Fate of Ophelia,” but she was also awarded the inaugural Artist Director Honors, which recognizes “groundbreaking music artists whose work behind the camera has expanded the possibilities of music video and advanced the art of visual storytelling.” By the time she had collected that prize, as well as two other Moonmen, Swift had become the most-awarded artist in the history of the VMAs (with 33 wins in total).

To commemorate her big night, Swift debuted the music video for her newest single, “Patient Zero,” which she also directed, and which she co-stars in alongside Dakota Johnson and Colin Farrell.

In the visual, Swift portrays a ghost, following Johnson’s character around and acting as her guide as she dates a toxic man (played by Farrell) who Swift’s character was married to when she was still alive.

It’s a haunting, stark visual that acts as a strong companion to the many themes at play in the lyrics. Of course, in typical Swiftian fashion, the video also features plenty of Easter eggs, which wink to the singer-songwriter’s past music videos and her overall creative universe. (It’s no surprise, given that Swift has apparently been teasing “Patient Zero” for over seven months now.)

The cemetery

The “Patient Zero” video begins in a cemetery, which immediately brings to mind one of Swift’s most iconic videos, for “Look What You Made Me Do,” which sees her rising from the grave like a zombie, next to her tombstone that reads: “Here Lies Taylor Swift’s Reputation.”

This time around, though, Swift has come back to life as a ghost, and flashbacks show her being emotionally tormented, and eventually killed, by her husband. Her grave in the “Patient Zero” video reads: “Beloved Wife and Daughter Gone Too Soon,” while her the dates of her life are: “1986–2022.”

Fittingly, the last time Swift’s character “died” in a music video was in “Anti-Hero,” which just so happened to come out in 2022. It seems Swift may be playfully continuing a thread from that visual universe.

Notably, the age of Swift’s fictional character at her death was 36, which is the same age that Swift is now.

The newspaper headline

Speaking of “Anti-Hero,” there seems to be another nod to that track toward the end of the video, when a newspaper headline flashes across the screen, revealing how Swift’s character passed away. “Heiress Dies of Mystery Illness, Husband Inherits Billions,” it reads, while the subhead adds: “Spouse Inherits Estate Amid Unanswered Questions.”

This seems to be a nod to one of the most memorable verses in “Anti-Hero,” where Swift sings: “I have this dream my daughter-in-law kills me for the money / She thinks I left them in the will / The family gathers ’round and reads it and then someone screams out / ‘She’s laughing up at us from hell!'”

Sadly though, in “Patient Zero,” the murderous party gets their way, so Swift has to find a way to get them back.

The “tolerate it” tableau

At one point in the video, Johnson and Farrell’s characters are seen fighting at opposite ends of a long dinner table, which fans have noted is similar to the tableau set that Swift used for her “tolerate it” performance on the Eras Tour.

The toxic character that Farrell portrays could certainly be compared to the one that Swift’s narrator is singing to in “tolerate it”: “If it’s all in my head, tell me now / Tell me I’ve got it wrong somehow / I know my love should be celebrated / But you tolerate it.”

In the past, Swift has even explained that “tolerate it” was inspired by Rebecca by Daphne du Maurier, which tells the story of a woman being haunted by her husband’s first wife. Coincidence? I think not.

The Aston Martin

As Farrell’s character walks into the home in one scene, he is seen stepping away from a gray Aston Martin that is parked in the driveway. Of all the cars he could have been driving, this feels like a purposeful nod to Swift’s The Tortured Poets Department track “imgonnagetyouback.”

In that song, Swift sings: “Small talk, big love, act like I don’t care what you did / I’m an Aston Martin that you steered straight into the ditch / Then ran and hid.”

The cameos

When Swift made a cameo during the Emmys early in September, fans quickly deduced that her pre-filmed skit provided a clue about Johnson’s appearance in the “Patient Zero” video. However, now, it seems that Swift may have been alluding to all the cameos in her newest visual.

At one point in that Emmys cameo, Swift perplexingly said a bunch of gibberish (“Saccharide, Aries, from the vineyard, north or south”), which fans now believe was a reference to all the stars in her “Patient Zero” video.

“Saccharide” could mean “sugar,” which also happens to be the title of Farrell’s Apple TV drama series, Sugar. “Aries” is represented by a ram, and Matthew Stafford, who plays for the Los Angeles Rams, makes a cameo in Swift’s video as well. So does Swift’s longtime friend, Cara Delevingne, who represents “from the vineyard” because her last name roughly translates to “from the vine” in French. Finally, Johnson could represent “north or south” because there is North Dakota and South Dakota. Get it?

When it comes to Swift, nothing is an accident.
